There are several options to simulate grain in Photoshop. The most obvious is the noise filter.

There's a problem with the noise filter however, it spreads the noise uniformly over the image. Normally grain is more present in the shadows then in the midtones and the highlights. So if you aim for a more realistic film grain, you will want to use a curve:

* create a curve adjustment layer that lowers the tones in shadows but preserves the highlight (you will need two or three points on the curve for the right shape)

* fill the mask with black ("hide all" mask)

* add a liberal amount of noise to the mask

This is a better simulation of grain because the noise is filtered through the curve, if you like. It is more visible in the shadows than in the highlights).

It's a technique I learned from Professional digital photography, incidentally one of the most interesting and challenging photography book I read in 2003.
